Prevention First

A tiered physiotherapy service built for primary care

Musculoskeletal conditions account for a significant proportion of GP appointments yet many can be assessed, managed and discharged entirely within primary care when the right physiotherapy expertise is in place.

Our service is designed as a tiered offer: deploy First Contact Physiotherapists for high-volume MSK demand, add Advanced Physiotherapy capacity for complex cases, or run both for full-spectrum coverage.

"The First Contact Physiotherapist role is a cornerstone of modern MSK care delivery in primary care. It ensures patients receive early, expert and appropriate care while alleviating pressure on GPs, reducing referrals and improving outcomes."

— NHS England, FCP Programme Guidance

One service, two levels of expertise

First Contact Physiotherapist (FCP)

The primary access point for MSK in your PCN. FCPs assess, diagnose and manage musculoskeletal conditions independently no GP triage needed. Band 7 or above, HEE FCP Roadmap compliant.

Independent MSK assessment and diagnosis Red and yellow flag identification (Cauda Equina, malignancy, inflammatory conditions) Requesting and interpreting investigations (X-ray, bloods) Rehab advice, manual therapy and medication recommendations Streamlined onward referral to orthopaedics, rheumatology or community physio Shared decision-making and self-management support Face-to-face, telephone or video consultations

Advanced Physiotherapist (AP)

An extended clinical role for complex, undifferentiated or high-acuity cases. APs operate at an advanced practice level, managing presentations that would otherwise require GP input or specialist referral.

Complex and undifferentiated MSK assessment Advanced differential diagnosis and clinical reasoning Independent prescribing (where qualified) Injection therapy and procedures Spinal pathology, inflammatory arthropathy and chronic pain management MDT clinical leadership and governance contribution Supervision and oversight of FCP colleagues

Why introduce FCP physiotherapy into your PCN?

Release GP capacity

FCPs and APs independently manage MSK caseloads that would otherwise sit with GPs up to 30% of appointments freed.

Faster patient access

Patients are seen promptly by the right clinician at first contact no GP appointment needed first.

Fewer Unnecessary Referrals

More MSK conditions diagnosed, managed and discharged within primary care reducing secondary care pressure.

Better Clinical Outcomes

Earlier, expert MSK assessment leads to more accurate diagnosis, safer management and improved patient experience.

Stronger MDT Integration

Both roles work closely with GPs, ANPs, social prescribers and secondary care expanding clinical capacity across the network.

Traditional model vs embedded FCP physiotherapy

Traditional model
GP manages MSK presentations alongside full clinical workload
Delays in specialist MSK input
High onward referral rates to orthopaedics
Complex cases escalated to GP
Limited MDT physiotherapy integration
Reactive, appointment-led care
Embedded FCP Model
Dedicated physiotherapy assessment from first contact
Same-day or rapid access for MSK patients
More MSK conditions resolved within primary care
Advanced Physiotherapist manages complexity independently
Physio embedded across the full MDT pathway
Prevention-focused, population health approach

Frequently Asked Questions

Many PCNs start with an FCP service to absorb high-volume MSK demand and add Advanced Physiotherapy capacity as caseload complexity grows. We'll help you understand which configuration suits your network's current need.

Yes. FCPs are autonomous clinicians who assess and manage MSK presentations at first point of contact patients can be directed from reception or care navigation without a GP appointment first.

All Xcel Primary Care FCPs meet HEE requirements: Stage 1 portfolio of evidence against core capabilities, verified at Stage 2 by a clinical supervisor or Advanced Practitioner ensuring a consistent, quality-assured standard across the service.

Where practitioners hold an independent prescribing qualification, yes. We can discuss prescribing capability as part of service design.

Yes. Delivery can be structured across one or more practices within your PCN to match demand and operational need, including remote, hybrid or face-to-face formats.

We agree reporting metrics collaboratively with each PCN, aligned to your population health priorities. Standard measures include GP appointment release, referral rates, functional outcome scores and patient satisfaction.
